Character: Noth Queha'ha, Lvl 4 Ranger
Adventure: Three Faces of Evil
Location: The Bridge
Cause of Death: Attacks of Opportunity

Noth was caught between a rock and a hard place. Specifically she was caught on a rope bridge, and the hard place was the 3 Grimlocks raining down javelins on her. After failing twice to retreat-that 10 acrobatics check is tough in heavy armor with 0 rnks--she took the 3 head-on. In an attempt to help her push the monsters off the ledge, our cleric enlarged her. It only took 2 bad rolls on repositions (we houseruled that you can reposition off cliffs but the repositionee has a +4 to CMD) for the grimlocks to rack up 6 Attacks of Opportunity and 6 regular attack. With her reduced AC she was down before any allies could even get halfway across the bridge.

Lucky for her (or her player anyway), the grimlocks kept her with just a smidge of life so they could drag her away, revive her and torture her later. She may come out of this yet.


Character: Kudish halforc druid 7

Adventure: Hall of Harsh Reflections

Location: Telakin's Lair

Cause: Critical AoO from Telakin barbarian form.

Kudish was living on borrowed time from the start of the campaign. It was amazing he made it this far. Kudish began the battle by sending his ac up one set of stairs and him up the other. Both failed their reflex saves for the pit traps. After climbing out of the pit he shaped changed into a tiger then jumped through a wall of fire cast by Telakin. By this time Telakin was in barbarian form and full attacked Kudish whom he easily dropped. The bard then healed Kudish for 20 hp. On Kudish's next turn he stood up and provoked an attack of opportunity. Confirmed critical from a raging barbarian with a great axe. Needless to say Kudish didn't make it.

The Death Trap lived up to it's name.

After being brought back with revivify, the Death Trap proved it was, in fact, perfectly titled.


I didn't take the time to read through every one of these, but I had a couple party member deaths, followed by a TPK.

Character: Kizzair (Level 3 Half-Orc Fighter)

Adventure: Three Faces of Evil

Location: Temple of Hextor

Cause of Death: Theldrick

Long Version: "Tank" with 25 AC, Full Plate, Tower Shield, the works, got hit with Hold Person right off the bat, then beat on by a fiendish dire ape... sadly enough, thing could barely hit him. Theldrick on the other hand had no problem smacking him in the face with Inflict Serious Wounds, followed by some more inflict spells.

Character: Silivren (Level 4 Human Sorcerer)

Adventure: Three Faces of Evil

Location: Cavern before chokers with the grimlock archers on the ledge

Cause of Death: Shot in the back + 40 Foot Fall = Pew Pew Dead squishy

Long Version: Just so happened that the tactics for the grimlocks were to wait until the 2nd to last party member climbs down, they used a rope to make the climb easier for the heavily armored, rogue was going to go last so that she could bring the rope down with her because she could just climb the spikes already given. Poor sorcerer.

Character(s): (TPK) Alayi (Level 5 Ifrit Rogue), Aelyria (Level 5 Elf Druid), and Aela (Level 5 Cleric)

Adventure: Assault on Blackwall Keep

Location: Lizardfolk "hatchery"

Cause of Death: Worms.... Lots of worms

Long Version: Everyone stayed in the water just a little too long, didn't clear the water and worms fast enough, they created spawns that got some lucky hits in and the poison dropped the rogue's Constitution to 0 quickly, became a spawn of Kyuss and ROFL-Stomped the others. Fast Healing is no joke. Meanwhile, Kahjira, the Druid's Pet wolf had to wait on the other side of the swim into the hatchery, where she awaited her master's return until dying of starvation.

This Campaign is deadly.

It feels like I've been running my Eberron version of AoW forever. :)

So many deaths, so many new PCs.

- My entire first party, sans the warforged fighter. They died to the Green Dagger Gang in Mad God's Key, or to infections of red death.

- Gentleman Jack, a changeling factotum that developed a fear of wolves after the starving wolves in the Whispering Cairn almost ate him. He fled the party after they had a high-noon showdown with Cubbin, and was killed by Filge.

- Garret Razorclaw, a shifer druidic avenger. He died from mummy rot in the Crypt of Crimson Shards.

- Killian d'Phiarlan, a Phiarlan elf ranger. He died attempting to deactivate the strange automaton in the Mud Sorcerer's Tomb.

- An unnamed Janni that died within the Mud Sorcerer's Tomb when the party asked him to try on a magical necklace. It's the only time I've seen a player die without ever rolling dice in-game, because that necklace of strangulation killed him dead-bang.

- Vash'tar, a human cleric of the Dragon Below. He fell victim to the party when a spirit naga within the Mud Sorcerer's Tomb charmed him.

- Raina, a Cyran warmage who fell victim to the symbol of death in the Mud Sorcerer's Tomb. She got better.

- Aerdane d'Lyrandar, a Lyrandar half-elf bard who trigged the aforementioned symbol of death. He also got better.

Now the party is making their way to the Spire of Long Shadows. Without a cleric. This is going to be good.

Pathfinder Adventure Path, Lost Omens Subscriber

The druid's beloved eagle animal companion was eaten by the mimics lurking within the Sodden Hold. This caused the druid to go insane with grief, and she wildshaped into a fire elemental and began burning the building down in her rage, not caring one iota for the mission anymore.

When the Witch (really a doppelganger who had replaced him) decided this was an excellent time to devastate the (critically low HP) split party with a fireball (and another), she snapped out of it and engaged in an epic aerial battle over the streets of the city before finally blasting the fleeing witch doppelganger from the air with a well-placed call lightning.

She reincarnated her eagle shortly thereafter - we crossed our fingers, praying it would come back as a giant hamster, but lo and behold, she brought it back as a freaking roc. Or as we called it, "Super Eagle."

The druid herself fell to a last-round scorching ray from Zyrxog, and her roc companion carried her out of the dungeon before the rest of the party could do anything about it. Her whereabouts are unknown.

When the rest of the party sought a new member, a wizened (and familiar) old man joined them at the Crooked House. As it turns out, the Rain Barrel Man was a true prophet after all, and when not enthralled by apocalyptic visions, he commands formidable power as an Oracle of Time.


Character: Sharissa Halfling Sorc 9
Adventure: The Champion's Belt
Location: Altar Room
Cause: Multiple channled negative energies.

After the death of Kudish the halforc druid, the party recruited the services of the blaster happy halfling sorcerer Sharissa.

While the party wasn't initially thrilled with the new acquisition, (being on the receiving end of several of her fireballs) they respected her ability to clear a room with relative ease.

The alliance however was cut short by one Bozal Zahol. While Sharissa was clearing out the room of Kyuss Spawn and a Mohrg, Bozal was erecting his formidable defenses.
When the party entered the altar room they were greeted by a small army of summoned earth elementals and an angry Babau demon.

During the battle Sharissa learned that Babau demons have no problems seeing invisible halflings as she took critical wounds from the demon.

Bozal not idle during the battle sat back in the comfort of his anti-life shell and spammed channeled negative energies.

Unfortunately when Sharissa fell to negative energy onslaught, her invisibility spell was still in effect. The party was unaware of her demise until she reappeared.

Perhaps the last two deaths in my campaign:

Character: Ellizabella Barriston, artificer 21
Adventure: Dawn of a New Age
Location: Lashonna's Ciborium
Cause: Vampires. Lots and lots of vampires.

Ellizabella (Lizzy) has been an almost unstoppable force throughout the entire campaign, an artificer with defenses beyond most beings ability to bypass. Lashonna, knowing this, equipped herself and her three blessed angels with antimagic torcs (from Underdark). Despite her penchant for spellcasting, she realized that her best option was to disable all the items and engage in melee against a defenseless Lizzy. Along with a blessed angel and three vampires, Lashonna cornered Lizzy and her level was drained entire after a couple of rounds.

Character: Keveth, fighter 21/paladin 11, hellreaver 10 (gestalt)
Adventure: Dawn of a New Age
Location: Lashonna's Ciborium
Cause: Blessed Angel

Not long after that, Keveth was attacked by a blessed angel that had not yet activated it's torc. It managed to unleash a full attack, and one lucky roll of a 20 allowed the vorpal sword to cleave Keveth's head off. Luckily for him, he had a contingent revivify and the next turn. From the ground, he grabbed his sword, which had been dropped in the adjacent square several turns earlier in favour of a wand, and stabbed up at the angel, channeling all of his abilities into it and slaying it in one strike.

This was supposed to be the finale, and Kyuss has just emerged from the monolith, but unfortunately a difficult scheduling situation and a thankfully harmless medical scare forced us to stop there. Wouldn't be surprised if I'm back here in a couple of days with one or two to report from my powered-up Kyuss.


This thread inspires me to do my very best in the PbP I'm about to start tomorrow.

Alas, We Hardly Knew Ye: E died when the spider-slasher monster got off a successful AoO on him, and then E was devoured by the acid beetle swarm.

So, we're off to a great start.


Thanks, Cat (and get off this subforum before you spoil yourself! ;P). To expand:

Name: Erip, level 1 genderfluid dwarf necromancer (and Lonsoc the rat familiar)
Adventure: The Whispering Cairn
Location: The lantern room with the beetles.
Cause of Death: Recklessness, dumped Dex and Con, and a swarm of hungry beetles.

When the mad slasher climbed out of the shaft, Erip and his/her friend, Tanith, were the first things it saw, and it made its way to threaten both of them.

Unwisely, Erip tried to cast grease while still within reach. His 8 AC and 6 HP both came back to bite him, and he was knocked unconscious at -3 (9 Con).

Before anybody could move to heal the dwarf, the beetle swarm, having trouble finding a target, decided the unconscious necromancer was an easy bet. It moved over Erip, dealing another 9 damage and ending Erip's life.

The next round, the alchemist threw a bomb, finishing off both the swarm and Lonsoc. Erip never got to cast more than a cantrip.

Two encounters. Two deaths. So far, so good. :)

Name: Robin O'Haranson
Race: Human

Adventure: The Whispering Cairn
Location: Lair of the Laborers, the Flooded Rooms
Cause of Near Death: Ghouls & Water: A Deadly Combination!

The party came upon the stairs leading to the flooded parts of the Lair of the Laborers, and ran into ghouls.

The Slayer was grabbed by the ghoul and dragged under, whereupon he tore it to bits.

Our erstwhile Hunter, Robin, thought it would be a brilliant idea to jump from the top of the stairs into the watery melee to assist him.

You see where this is headed?

Well, there was another ghoul that was hiding where Robin landed in the water. It promptly critted him twice: once with its bite, and once with a claw.

The end result was that Robin was dropped to -3 hit points and drowning. If it weren't for the warpriest, Tanith, hauling his butt out of there, and for the new cleric of Olidammara channeling energy to stabilize him, he'd be dead.

And KC, relax. I'm not reading upthread. :)

The Gory Details: The party was pretty beat up by the time we reached the main chapel of Hextor's temple. In the ensuing melee, everyone gave as good as they got. I still remember the Hail Mary bomb I nailed Theldrick with.

Unfortunately, despite no-selling his hold person, disrupting another of his spells, and dodging countless arrows, Carina Viera was finally laid low by the dying shot of a tiefling guard (which, honestly, was pretty badass) and slain by one of ThelDICK's spiritual flails.

To her credit, she dodged TWO of them like a boss before she was slain.

The Overgod realized that if he wanted to stop the paladin and fighter from beating on him, he had to kill to the cleric who was healing them. 4 attacks, 2 of them being criticals were enough to drop her into negative health points. Unfortunately, Eydis rolled a 1 on her stabilization check and died. She was brought back soon afterwards.

The battle with Zyrxog was hanging by a thread after the death of half the party. Our BSF was mind blasted immediately and the Magus was "suggested" out of the battle shortly thereafter, eliminating the 2 primary damage dealers. Kraldar and Scruffin had trouble beating spell resistance so they tried melee even though Kraldar is only so-so at it.

An aerial battle ensued and Kraldar fell first, being whittled down by lightning bolts and scorching rays. Scruffin, seeing the battle would be lost, tried to run. A well-placed empowered lightning bolt killed her outright.

Lucky for the party, Zyrxog was overconfident because he basically owned them the first few rounds. He didn't notice the BSF come out of stun, and a VERY fortuitous critical hit did 50 damage and turned the tide of the battle.

Kraldar is survived by Amadi Alo Arustan, the BSF that he rescued from the streets.

Scruffin is survived by her Dweomercat familiar, Pokey.

Following a lead the adventurers went after a group of high level Ebon Triad cultists in a neighboring country. In the climatic final battle, Eydis was hit by a maximized scorching ray (3 rays, one of which crit). She once again managed to roll a 1 on her stabilization check and died. She has since been patched up.

Cause: An angry tree

In the fight with the Night Twist in the Doomshroud forest, Ali had managed to stay near the tree and was smiting it for a bunch of damage. The tree full attacked her back and did something like 165 points in 1 round. She was subsequently resurrected as the party continues the trials.

Cause: Swarms of Swords of Kyuss

During the dream sequence at the end Hefe was holding off a squad of Swords of Kyuss while others dealt with the Boneyard and the other undead. He went down under a hail of power attacking greatsword hits and crits. He was rezzed after the boneyard was defeated and the party completed the dream sequence. The group is now off to the "King of the Rift".


Character: Sir Nathanial, human paladin 15 (mythic champion 1)
Campaign: Age of Worms

Cause of Death: After being horribly wounded by Darl’s efreeti bodyguard, he was final cut down by the same fire genie. His ally Gimpy restored him to life the next round with a Breath of Life Spell.

Character: Clive, half elf wizard 15 (mythic archmage 1)
Campaign: Age of Worms
System: Pathfinder
Adventure: Library of Last Resort
Cause of Death: Clive got a little too close to Kranthanos the titan, and was hit by a blow from the titan’s hammer that killed him instantly (greater vital strike).


Wow, I've read so many of these and I finally have my own. RIP Gunda. Hopefully your replacement is a bit more careful.

Character: Gunda, Gnoll Barbarian 1
Campaign: Age of Worms
System: 5th Edition D&D
Adventure: Whispering Cairn
Cause of Death: Underwater Ghoul. On a return trip to the Whispering Cairn, a slightly injured Gunda went down into the submerged passages alone and was ambushed by the ghoul. The fight came down to the wire but with his HP just a little down from full the ghoul killed him before he could finish it off. Sometime later rest of the party followed and neatly bisected the ghoul as it fed. Gunda's corpse would be burned to prevent ghoulification.


Character: Sharlto.
Class: Paladin 18/Oracle 1/Holy Vindicator 1.
Adventure: Into the Wormcrawl Fissure.
Cause: Nonchalance. If you're in the Tabernacle of the Worm, it pays to cast death ward. Sharlto pushed through a wormdoor and was surprised by the 6 avolakia on the other side. How many enervations does it take to kill a 20th-level character? Turns out it's 9 (6 in the surprise round and, after he lost initiative on his first round, he was hit by 3 more).

Character: Sloane.
Class: Alchemist 10/Master Chymist 10.
Adventure: Into the Wormcrawl Fissure.
Cause: One bad roll. Having found the Book of Vile Darkness, Sloane had a quick peruse and failed the (fairly easy) Will save. He took some damage and thought nothing more of it, even after the wizard informed the party that the book can turn the reader evil.
The party continued adventuring and finally encountered Dragotha. After some dialogue we rolled initiative. When Sloane's turn came up, he declared that he would delay until some buff spell was cast. I then told him that he was in fact going to attack the party.
After I explained what had happened (evil book, you're evil, change sides, etc), he dropped Sharlto (see above) into negative hit points in one round. The rest of the party grabbed the unconscious paladin and teleported away, leaving Sloane to chat with Dragotha.
Shortly after, Sloane was transformed into a Favoured Spawn of Kyuss, which counts as dying in my book.

Technically, a vampire can't level drain in an anti-magic field, as energy drain is a supernatural ability.

Well, that's good to know. I'll have to let the player know that he was even more impossible to kill than I'd thought. :P


Character: Daln, Bringer of Light
Class: Cleric of Pelor 5
Adventure: Three Faces of Evil
Cause: Not having enough HP

Oh Daln. Played by the DM (me), he prefaced every single sentence with nothing but "By Pelor's Light..." and basically served as a HealTank. This didn't work out so hot with the Overgod, who brutally beat him to death. I let him live for the rest of the encounter (it was late at night and I was tired), which only ended when the PCs killed the Overgod by dropping the elevator on it as it scaled the shaft to pursue them while they fled. Best. Encounter. EVER.

Character: Samaki Broodsdotter
Class: Rogue 8
Adventure: Hall of Harsh Reflections
Cause: Killed by Self

Poor Samaki was the other DM played character. He was constantly abused by the rest of the party, who often ended up accidentally nearly killing him by catching him in AoE, or sending him into melee despite his woefully underpowered character. This ended when he was cut off in the mirror maze by a wall and attacked by a doppelganger version of himself. The party asked them a question to determine which one was real. The fake Samaki shrugged, and stabbed the real one through the chest, dropping him from about 17 to -17 in a single hit.


In addition:

Character: No one could remember his name.
Class: Soulknife 9
Adventure: Hall of Harsh Reflections
Cause: Lots o' lightning

He was played by an enormous powergamer, who wanted to swap classes to swordsage. I told him he couldn't just magically swap classes, and had to swap characters in some other way. Zyrxog brutally blasted him with about 80 metamagicked spells, problem solved. Now the only remaining member of the original group is the party wizard.

BONUS: Wizard's familiar.
Class: Fire-breathing dragonborn rat
Adventure: Hall of Harsh Reflections.
Cause: Deep fried

The wizards character was tied to a chair and being repeatedly blasted by Ixaxian, who had his wand of burning hands. The rat got caught in AoE, and burnt to a crisp. The wizard made it out with negative HP, but was Ok.


Well, Erip's player hasn't had much luck, has he? This time, he's playing the aforementioned priest of Olidammara. But first, a brief mention: The aforementioned tiefling alchemist Carina Viera, played by Dragoncat, has been semi-recently reincarnated into the form of an undine.

Viera was alive, at the beginning of this adventure. On this matter you must be clear, or nothing wonderful can come of the story I am about to tell you.

Name: Astraden Limhaare
Build: Human Cleric of Olidammara 6
Adventure: Hall of Harsh Reflections
Catalyst: Invisible stalkers and an ex-tiefling

Everybody knows how nasty those stalkers are. Well, it should come as a surprise to nobody that they've been a reeeal handful for this party.

The encounter's still playing here. Two of the four* stalkers are dead, but the remaining two are still causing trouble. The party's slayer is unconscious, and the cleric was at 3 HP up until a round ago.

This round, the aforementioned Carina threw a bomb down. Unfortunately, it was a crowded area, and the cover was her undoing. Barely. The bomb went wild and hit Astraden. Though she made her save, this knocked her down to 0 HP.

The next round, the stalkers decided to bring her down. She'd proven a troublesome factor, healing their targets and recently killing one of their own with a nasty Elemental Channel.

One of them full attacked her. The first slam missed, but the second hit. Now, even from here, there still wasn't too much danger. The stalkers deal 2d6+4 damage and she has a 13 Con—they could only kill her with an above-average roll.

They rolled a 14 for damage, killing the spirited priestess by 2 points. RIP, Astraden.

*Pathfinder party of six, so all encounters end up pretty beefed up. I try to keep things level with the original CR/APL discrepancies.


Poor Gark. He's having trouble with his PCs.

Name: Gattel Watam
Build: Human Investigator 5 (I told him to level himself up with the rest of the party)
Adventure: Hall of Harsh Reflections
Catalyst: Doppelpunches and having a heart two sizes too big.

When Astraden died, I told her player he could take over Gattel Watam, the crazy old tax collector whose brain got fried by the mind cloning process. Restatting him as an investigator, he joined the party and quickly made himself useful by providing valuable comic relief (and the odd bit of crucial information, thanks to his good skill selection).

When the party found Filge, Gattel's enlarge person extracts were what confirmed that Filge was still Filge. When the party slayer was fighting a doppelorc in the closet, Gattel put an arrow in the orc's forehead. Gattel may not have been the most powerful member, but he unquestionably helped.

Unfortunately, his time ran out in the Hall of Deception. The bard at last revealed himself as the traitor, and when a nasty command spell sent the barbarian fleeing through five AoOs, Gattel was the first to run after him, heedless of an AoO sneak attack from the vanished Ixiaxan. He stabbed the barbarian with a healing syringe, keeping the mighty Olman up another round.

This would be Gattel's last act. The next round, the doppelgangers rushed him. One hit would have been enough, but they scored a critical.

With a sickening crack, Gattel's neck spun 'round, and that was the last anyone heard of the poor old tax collector. He never did get his mind back in one piece. Nor did he get those doppelgangers to pay up. Perhaps the adventurers he helped will be able to finish what they started.

Catalyst: Doppelganger crit.

As previously mentioned, Cuetzpalli had been driven into danger by a command spell. Down to 2 HP, he'd been brought up to 18 by Gattel only to be reduced to 6 by another blow from a doppelganger monk.

The next round, he butchered his double. Unfortunately, two more doppelgangers were attacking him. The first two attacks missed. The third got a 20.

Cuetzpalli's with Astraden now. He is survived by his dinosaur companion, "Spiny".

I don't want to give too much away. Let's just say that the poor guy took over 1600 hp damage before he even cleared the monolith.

Cause of Death: Alkilith's cone of cold

Cause of death could easily also be listed as "Party streches themselves too thin".

The heroes fought pitch blade, killed a bunch of kyuss spawn and fought bozal and his summoned goods . . . all in one day. That would've been a good time to call it a day.

But they pressed on, sure to find a switch or key that would turn off the scrolls and kill the giant worm monster. What they found was a hostile demon who was just a bit too much for the already-drained party. The cleric panicked at the sight of the kyuss mosaic. Hess was nauseated by a stinking cloud. The remaining 2 fought hard but were beaten and had to retreat. Hess had barely finished cleaning herself up when a cone of cold caught her off guard, making her a permanent corpsicle.


For awhile we didn't have many deaths and I thought maybe I was doing something wrong. Then the party got to the Free City in HoHR and since then everything has gotten a lot more fun :)

Cause of Death: Madtooth

Amadi didn't really stand much of a chance. Boldly, he charged madtooth on round 1, and sadly, he missed. From then on it was a pummel-fest: Madtooth hit on 9 of his next 11 attacks. In a last ditch attempt to save Amadi, our sorcerer enlarged him to prevent him from being swallowed whole. It worked. Sort of . . . Madtooth instead tried to chew him up. The last bite attack killed him.

Amadi's death was not in vain though. He kept madtooth busy long enough for the rest of the team to whittle away his HP. Everyone else survived completely unharmed.

Cause of Death: Full attacked by Xyzanth

Xyzanth had noticed the party chop up a bunch of his compatriots, so he sent several dragon minions to distract the party while he came in and launched his attack. The first few rounds of combat found the fighter drained of 7 con and down 3/4 of his health, the cleric had been taken down to about 10 hp left and had tried to teleport away (she rolled a 93 and appeared at the other end of the rift.
The rest of the party was mostly ineffectual, as they were trying to take down the dragons buffs so as to actually hit the thing. Ali was the only one left in melee and managed to last 8 rounds before succumbing to a full attack by the fang dragon.
Unfortunately for the dragon she had a Blaze of Glory spell up, which went off for 146 points of damage*. thus healing the party and grievously hurting Xyzanth. The fighter lopped off the dragons head off the next round. Ali was True Rezzed back.

* Our version does 2d6 per level, max of 30d6 (double vs undead). The paladin however does not fall unconcious, but rather is disintegrated.

Moved to help out vs a Fire Giant ranger and forgot about the giants reach. Was brought back the next round with a Cure Deadly spell.

While simultaneously fighting the frost giant sorcerer and the marilith, Caine was unfortunately the next target in line for an implosion spell. As we ended for the night after hat fight, his fate is TBD.

Name: Serrif
Class: Human Fighter 14
Adventure: Spire of Long Shadows
Location: Sea of Worms
Cause of death: Overworm and several Flame Strikes.

Taking the lead, Serrif and Mantis flew over the Sea of Worms to scout. There they encountered the wormcallers, who began harassing the party with spells. Serrif landed on a small island to attack a wormcaller not knowing that a gargantuan worm lurked near his location. Said worm then burst to the surface landing a massive hit on Serrif. Turning their sights on this new foe, Serrif and Mantis slay the overworm. Their victory was short lived as the two wormcallers each dropped a Flame Strike. The ranger easily avoided both spells, the fighter however did not. The first spell knocked Serrif unconscious, the second was fatal. The only luck Serrif had was that he was still on his small island when he fell.

Entering the Night Twist's clearing, the party was immediately assaulted with spells meant to turn their worst fears against them. Undaunted by their fears, Seriff and Mantis close in while the party's bard and inquisitor cast buff spells as the evil tree cloaks itself in Deeper Darkness. The bard successfully dispelled the darkness, and Serrif moved into melee scoring a critical hit. Then everything went wrong for Serrif as the Night Twist went: hit, critical hit, critical hit, hit, hit and hit dealing +/- 280 damage

Climbing the cliff the unlucky wannabe sapphire hierarch was critically hit by an arrow dropping him to -2, the 40ft fall on to sharp rocks did the rest.

The acid beetle swarm appeared. She tried to delay it to let the others retreat and managed... She died in 2 rounds. The summoner's ponies kept them back longer allowing the rest to escape.

The Three Faces of Evil is still well ahead in confirmed kills and even broke 200, but Spire of Long Shadows continues to ascend. Hall of Harsh Reflections and Champion's Belt entered a dead heat, but while Champion's Belt is likely a bit deadlier, HoHR's earliness in the campaign (and thus greater accessibility) gave it the edge it needed to pull ahead of Champion's Belt and SoLS.

We saw a real push from later adventurers, especially Library and Wormcrawl Fissure. Just when Spire would start to lag behind, another big TPK would boost it up back to #3. Odds are there's another one on its way even now. Notably, there was one incident a while ago where a party got almost entirely feebleminded inside the Spire, but I didn't count that since no deaths were noted. Spire is steadily climbing, though (TWC grew 112%, while SoLS grew a whopping 150%), and I'll bet that the next time we update, Spire of Long Shadows will be at #2 to stay. That adventure is nasty.

Come to think of it, my own party is coming up on the Spire now...

Oh! I made a graph for you guys! Edits are off, since otherwise updates would get confusing.

On the 2nd night out from Diamond Lake, the Ixit, the party druid, and Vanthis were on watch when they noticed the horses growing restless. They went over to check them out, and perhaps try to calm them. What they failed to notice was a half dozen owlbears approaching.
Ouch.
The party put up a valiant fight, but in the end, all of the horses were slain.

Oh, and Vanthis died too.

The Exchange

Our wonderfully buffing bard Jullienne came to our party/
at the beginning of the Hall of Harsh Reflections;
A great addition were she and her ranger cohort/
Not to mention her dragonfire inspirations.
But alas her visit, it was fleeting/
As we reached the end of the Hall;
The wizard chose to not Resist Energy/
And everyone failed the save against Zyrxog's Fireball.

We will miss her.

PC Name: Astraden Limhaare (You might remember her)
Adventure: The Spire of Long Shadows
Location: The Hall of Knowledge
Catalyst: Invocation of the Worm and a bad Reflex save.

Poor Astraden can't catch a break. The group opened the door and all but one of them lost initiative. The Swords of Kyuss stepped up (one took a readied Shield Slam from the one PC who won initiative, but kept coming) and loosed their Invocations. Astraden only has a +3 Reflex save, and it's DC 22. She needed to make one save to survive, and blew three Action points to do so. She got a 21 on her second save. Close, but no cigar.

Actually, this death was retconned shortly after by the bard's moment of greatness, but it did still technically happen for all of 15 minutes. You can see it here.

Still waiting on one potential death from the invocations, so more entries might be up soon when more saves are posted. Ugly stuff.

The party attacked Thessalar head on, fighting him and his hydras at the same time. Thessalar dropped a Wail of the Banshee on the group, and Alax managed to roll a 1 on his save. He was True Rezzed after the fight was over.

Class: Human Bard 11/Ranger 1
Adventure: The Spire of Long Shadows
Location: The Fountain of Green Temptation
Catalyst: A really bad day

Eben had already annoyed the three wormcallers by dispelling their buffs. The next round, they singled in on him, firing three ribbons of maximized shadow. All three hit him, and the third did almost max damage.

He took a total of 118 damage and was killed instantly. The next round, the alchemist stabbed him with a breath of life syringe, and he was immediately compelled to drink from the fountain.

As the party moved into temple square, they were be set upon by a group of 12 Knights. Hefe was holding down one flank and was finally overcome by the intelligence draining eyes of the knights. Once he collapsed he was Coup-de-graced the next round. He was true rezzed after the fight.
